This page contains all the forms you need to participate in athletics at our school. Please read the instructions carefully before filling out any forms. If you have any questions, please contact the athletics office.

Athletic FORMS for Participation

We are making a change in how we host and maintain your student-athletes' paperwork because Big Teams is merging with ArbiterSports. Northside will no longer use the Big Teams platform, effective at the end of the current school year. This change affects Northside and many other schools. Parents should no longer update any paperwork on Big Teams.

Every athlete will need to create a new account on ArbiterSports in the coming weeks, but we are **still working on this process**. Please hold off on registering your athlete until you receive an email from me with additional instructions.

As a reminder: All student-athletes will need a current physical signed by a doctor for the 2026-27 school year. Let me know if you have any questions.
